  • Mailing address
  • Health history

2. Make a List of Your Physicians & Hospitals

  • Physician name(s)
  • Hospital name
  • Phone numbers
  • Locations

3. Estimate Your Health Expenses

  • Ballpark upcoming doctor, hospital and ER visits
  • Anticipate new health events (starting a family, major dental work, etc.)
  • List your Rx dose, quantity and current co-pay

4. Determine Your Top Priorities

  • Lower co-pay/co-insurance
  • Lower annual deductible
  • Lower monthly premium
  • Lower prescription drug costs
  • Specialized drug coverage
  • Keeping your doctor/specialist
  • Coverage while traveling

5. Factor In Your Budget

  • How much do you have in savings (HRA, HSA and/or FSA)? You may be able to handle a higher deductible
  • Do you visit your doctor a lot? Consider a higher premium but lower co-pays for office visits
  • What did you spend on health care last year? You may need help with high recurring costs like Rx
